Within a non-reliabilty aware company a correct application of the recommendations collides with the own way of working. One was reluctant to change. Nevertheless reliability is a top-down driven activity, just like quality. Courses given to the top did not trickle down to the bottom-up. There were only small scale changes with no cohesion.
Little successes were reported. But winning a battle is not winning a war. Unreliability is a war. Year after year with no real improvement is frustrating. Stucked into old fashioned way of working. There is no breakthrough wanted but a breakout. Declare war to unreliability, get the right armour and start. Don't relax after winning a small battle.
Reliability is no matter of formulas and procedures. It is a matter of common sense. It must be between the ears. It is a way of life. Formulas, procedures and so on are just tools.
